is checked for the required fields. After the form is verified to be correct, it
is sent to an e-mail address that only your practice has access to. You schedule
the appointment and send a confirmation via e-mail to the client indicating appointment
date and time.
Return Form
If a new client is scheduling a doctor appointment, they can check the new client
checkbox. A Microsoft Word and PDF form will be attached to an e-mail and sent to
them automatically. They can fill it out regarding their pet’s past health history
and either send it back as an attachment in an e-mail or bring it in with them on
their first visit.

The Boarding Reservation Request Form provides a means for clients to request a
boarding reservation via e-mail. The client fills out their name, address, e-mail,
phone number, pet’s name, length of stay, and desired time to check-in their pet
for boarding in an on-line form. Once the client presses the
Send Boarding Request button, the form is checked for the required fields.
After the form is verified to be correct, it is sent to an e-mail address that only
your practice has access to. You schedule the reservation and send a confirmation
via e-mail to the client indicating the reservation date and length of stay.
Return Form
If a new client is scheduling a boarding reservation, they can check the new client
checkbox. A Microsoft Word and PDF form will be attached to an e-mail and sent to
them automatically. They can fill it out regarding their pet’s behavior, diet, and
demeanor towards other animals and people. They can then send it back as an attachment
in an e-mail or bring it in with them when they check-in their pet. This will speed
up the check-in process.

The Grooming Appointment Request Form provides a means for clients to request a
grooming appointment via e-mail. The client fills out their name, address, e-mail,
phone number, pet’s name, type of cut, groomer preference, and desired date and
time for a groomer appointment in an on-line form. Once the client presses the Send Grooming Request button, the form is checked
for the required fields. After the form is verified to be correct, it is sent to
an e-mail address that only your practice has access to. You schedule the appointment
and send a confirmation via e-mail to the client indicating the appointment date
and time.

history. A disclaimer form can also be attached to the e-mail if desired. They can

The Prescription Reorder Request Form provides a means for clients to request a
prescription refill for their pets. The client fills out their name, address, phone
number, pet’s name, medication(s), and date/time they will pick up the medication.
Once the client presses the Send Refill Request
button, the form is checked for the required fields. After the form is verified
to be correct, it is sent to an e-mail address that only your practice has access
to. You fill the prescription and send a confirmation via e-mail to the client that
the prescription is ready for pickup.
The Questions and Quotes Form provides an means for clients to ask a general question,
doctor question, or request a quote for a service. The client fills out their name,
phone number, e-mail, and question. Once the client presses the
Send Question Request button, the form is checked for the required fields.
After the form is verified to be correct, it is sent to an e-mail address that only
your practice has access to. You answer the question or quote and send an answer
via e-mail to the client.
The New Clients page provides a means for clients to download a form that they would
normally fill out in your animal hospital or clinic. New clients can fill out the
form at home rather than at your office. These forms are described in the corresponding
tabs as the Return Forms. The New Clients page is beneficial for new clients that
phone in a request rather than using your website to make a request. You can refer
them to your website to download and fill out the forms out at home in order to
save time.
